Downtown East Toronto HART Hub
The Regent Park CHC HART Hub is co-led with Fred Victor and the Downtown East Toronto Ontario Health Team (DET OHT). This HART Hub is designed to provide comprehensive support to individuals by offering primary healthcare, intensive case management, and other services. This Hub operates using a Hub and Spoke model, with Spoke services available at eight partner locations.
