East HART Hubs

East Toronto Hart Hub

The SMHTO Day Program is part of The East Toronto HART Hub, which provides evidence-based healthcare, treatments, social services, and housing support for individuals struggling with drug use, mental health, housing and other related issues. The Hub is an initiative of East Toronto Health Partners Ontario Health Team (ETHP OHT) led by South Riverdale Community Health Centre and St. Michael’s Homes with support from  Anishnawbe Health Toronto, Gerstein Crisis Centre, The Neighbourhood Group, Michael Garron Hospital, Alpha House, Fontbonne Ministries, The Salvation Army, 2-Spirited People of the 1st Nations, Woodgreen Community Services, Comprehensive Treatment Clinic and The Neighbourhood Organization.

  • Mental Health and Substance Use Counselling-Case Management 
  • Housing Case Management and Supports 
  • Psychotherapy 
  • System Navigation and Referral Support 
  • Voluntary Trustee Program 
  • Help with Basic Needs in a Safe Space 
  • Psychoeducational Groups 
  • Recreational Groups

East Hart Hub partner agencies can use this application  and select East Hub referral from the drop down menu.

Clients can also walk into the drop-in program at 688 Coxwell Ave to complete an intake
